There are three levels. Level 1 Support Connection builds your ability to link with services. Level 2 Coordination of Supports helps you design and coordinate the mix of supports and build capacity. Level 3 Specialist Support Coordination is for people with more complex situations and focuses on removing barriers and stabilising supports.
Your planner or LAC can include it when it is reasonable and necessary for you to pursue your goals. It is usually funded in the Capacity Building budget, and the plan may state the level you can purchase. Discuss your needs at your planning or plan reassessment meeting.
They should help you find and set up providers, coordinate services so they work together, coach you to build skills, solve issues, and report on progress toward your goals. The aim is to increase your capacity and confidence to manage supports over time.
You can search for registered providers by the relevant registration group and you can change coordinators at any time by following the notice terms in your service agreement. Your current coordinator should help with a smooth handover.
